Community Support Service

Why we need your help

Whilst we will continue to provide the crucial face-to-face contact of the community clubs, our Community Support Service will offer other ways to tackle social isolation, extending our reach to those who are unable to attend clubs; particularly the most clinically vulnerable and / or hardest to reach.

We offer:

• A TELEPHONE CONTACT SERVICE;  personalised one-to-one support that focuses on helping people to help themselves; e.g. supported signposting to take that first step to access services in their community,.

•VIRTUAL CLUBS; with a varied virtual activity programme; in addition to 2 current virtual choirs, 8 multi-activity clubs and service-wide events, we want to offer something different to attract new members which is not reliant on someone’s geography; health and wellbeing clubs, dementia-friendly concerts; quizzes for people who like friendly competition, particularly men, sports/music fans.

•DIGITAL COACHING:  1-to-1 digital support/loaned tablets and 2-4 week ‘new-to-zoom’ programmes in small friendly groups to support the transition between 1-to-1 digital coaching and virtual clubs.

We anticipate the new Community Support Service benefitting an estimated 150 older people in year 1, to increase by 50 per year, to reach a target of 250 in year 3. Our focus will be to support older people facing one or multiple challenges such as health issues, extreme frailty, bereavement, lack of family/social connections making them more vulnerable and less resilient in times of crisis.
